Initial steps
Before you submit a request, please ensure the following steps have been completed.
- Receive department approval (dept. curriculum committee or chair) prior to submitting your request to the committee.
- Ensure your request meets the College’s and UF’s requirements.
Once you are ready, submit your request by completing the appropriate College Submission form found below. Please note: not every required form is available on this site. Contact April O’Neal if your required form is not listed.
