I need to create a new course
- Obtain course approval from your Chair or Department Curriculum Committee. If applicable, obtain approval from the director of the program impacted by the course addition.
- Create new course syllabus using the PHHP Syllabus Template (below). Please remember that syllabus must be in the most current version of the College syllabus template (below) and meet UF standards.
- Review the PHHP Syllabus Item Coverage and Qualitative Review Checklist to be sure you have addressed the issues most commonly flagged during the approval process.
- Complete the New Course Submission form.
- Attach the course syllabus and if applicable, the UCC External Consultation form (link to form below).

I need to modify a course
- Modify current course syllabus using track changes and ensuring that the course uses the current PHHP Syllabus Template (below). Please remember that syllabus must meet PHHP and UF standards.
- Review the PHHP Syllabus Item Checklist (below) to be sure you have addressed the issues most commonly flagged during the approval process.
- Complete the Modify/Terminate Course Submission form.
- Attach the current course syllabus, the new modified syllabus (with track changes), a rationale for the requested changes and, if applicable, the UCC External Consultation form.
I need to close a course
- Complete the Modify/Terminate Course Submission form.
