Modify a Graduate or Professional Concentration

This form should be used to modify an existing concentration in a professional, graduate or interdisciplinary major, including changing the credits or curriculum of a concentration. Submit one request for each degree level. For example, submit two requests to change a concentration at the master’s level and doctoral level. Submit a single request to change a concentration in multiple degrees at the same level (example: M.A., M.S., M.S.T.).

Thesis or Non-Thesis(Required)
Is this concentration for a thesis or non-thesis degree?
Enter the name of the concentration to be modified.
List all the department / degree / major combinations at the degree level offering the concentration.
Describe the proposed changes to the concentration. If requesting a name change please provide details here as well.
Describe the rationale for the proposed changes to the concentration.
Describe any potential impact on other programs or departments, including increased need for general education or common prerequisite courses, or increased need for required or elective courses outside of the existing program.
Describe the Student Learning Outcomes and/or program goal data that was reviewed to support the proposed changes.
Describe the modifications to the Academic Assessment Plan that result from the proposed change.
Catalog Copy(Required)
Submitter agrees to prepare and upload document showing the catalog copy with the current and proposed curricula edited using the “track changes” feature in Word.
Please upload a catalog copy (catalog-style description of the specialization, requirements for entry, requirements for completion, and a suggested semester-by-semester plan, prepared as a Microsoft Word document) and any additional supporting documentation (memos, emails, etc. from other units to provide evidence of the availability of courses that are required in the specialization and/or to provide evidence for support of the proposed specialization if there is clear or potential overlap or duplication of content).
